Office Information: About Your First Visit
We accept most major insurance plans.
Please call the office to make an appointment in advance. If you are unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery.
A copy of your drivers’ license and insurance card(s) will be made at your first visit. Please bring any radiology films or CD and/or recent diagnostic reports, a current list of medications and any medical records with you at your visit. Any medical records that pertain specifically to your injury/pain that would be helpful should also be brought with you.
Fees & Payments
We make every effort to decrease the cost of your medical care. Therefore, we request payment arrangements for all office services at the time they are rendered unless prior arrangements have been made. We accept cash, checks, MasterCard, Visa and Discover for your convenience. If we are a participating provider of your insurance company, we will bill them. However, payment is the patient’s responsibility. We will help in any way we can to assist you in handling claims.
If you have a co-pay, it will be collected at the time of the visit. If financial agreements need to be made, please contact our billing department at (219) 924-3300 prior to your visit. We request that you notify the office of any changes with your insurance coverage, legal name, marital status, address or telephone numbers.
Prescription & Renewals
Please request all prescriptions and authorizations for renewals between 9:00 am –
12:00 pm, Monday to Friday, when our full records are available. Renewals requested at other times will be filled only for extreme and emergent circumstances.
